Note: use this form only if you are requesting one or two users who are not students or employees of the seminary. For groups of users, please follow the instructions below.

Students and faculty of Bethany Theological Seminary and Earlham School of Religion are given Moodle user accounts automatically. If you don't remember your password or can't log in for whatever reason read this first.

Others who wish to have access to Moodle will need to have a seminary faculty member request an account for them using this form. They should explain why you need access to Moodle in the comments field.

If you need to request accounts for several users, send the requested information in an email to help@scs.earlham.edu. Use the following format:

lastname,firstname,email<return>

Note that there are no spaces, that there is a comma between each item (you may use a tab or semicolon in place of the comma if you wish), and that there is a hard return between each user. Make sure your full name appears somewhere your email. Please include a contact phone number.

You will be notified by email when the new account(s) have been set up.
